How someone plays the

Thumb piano, is with their thumbs.

Now, there are some cultures that also use two fingers, along with the two thumbs. However, it also depends on the design of the particular instrument in question. With the very basic design it's just the thumbs, and you pluck the keys. You usually find they have a 3 octave range. Although, they can have different pitch ranges.
